2. Develop a list of questions that cover the key aspects of health and sanitation problems in the city. For example, the questions may cover topics such as waste management, air pollution, water pollution, public health facilities and so on. 

3. Decide on the sampling method and sample size. The sampling method may be random, stratified or purposive sampling. The sample size should be large enough to provide a representative picture of the population. 

4. Collect the data. The data can be collected through various methods such as face-to-face interviews, phone surveys, online surveys and so on. 

5. Analyze the data and develop a report. The report should summarize the findings of the survey, including the major reasons behind environmental problems, the most affected areas, the places that are clean, the areas facing major health problems and the government or citizen bodies or organizations that have been appointed to improve the situation. 

6. Share the findings of the survey with relevant stakeholders such as the local government, public health agencies, non-governmental organizations, and the general public. The findings can be used to develop strategies and policies to address the health and sanitation problems in the city.
